Neurofibromatosis (NF) is an autosomal dominant disorder of which manifestations are cafe-au-lait spots, depigmentation, neurofibromas, iris-Lisch nodules and mental retardation. Ischemic strokes associated with NF type 1 (NF-1) have been re-ported frequently. But, among these cases, cerebral infarction with complete occlusion of the extracranial internal carotid ar-tery was rarely reported. A 49-year-old woman with NF-1 suddenly developed right hemiparesis and dysarthria, one day be-fore admission. A brain magnetic resonance imaging (MRI) revealed acute infarction in left striatocapsular area. Cerebral angiography was carried out, and complete occlusion of the left extracranial internal carotid artery was noted. There were also shown moyamoya vessels on right carotid arteriogram. We report a case of moyamoya syndrome and complete occlusion of the extracranial internal carotid artery associated with NF-1.
